Every regulated industry keeps at least one document that exists purely to be doubted. The bank’s transaction journal, the hospital’s medication administration record, the airline’s maintenance log. None of them are written to be read on an ordinary Tuesday. They are written for the single day somebody asks what happened, in a room where at least one person would prefer a different answer.
Ask an AI vendor whether their product keeps an audit log and the answer is always yes. Ask a second question, and the ground moves.
Who is able to change it?
Consider a building with a security camera, a locked equipment room, and one night guard who is also the only person with access to the recorder. If that guard opens the room at 2am, the tape will show it, unless the guard decides otherwise before the shift ends. Nobody would call that an access control. It is a courtesy extended by the person being watched.
Most application logging is built exactly that way. The system that performs the action owns the record of it, writing with credentials that can edit and delete as easily as they insert. On a good day this is invisible, because nobody has a reason to rewrite anything. Audit controls are not designed for good days.
Four Ways an AI Log Fails the Only Test That Matters
The failures below are not exotic. They are the normal state of enterprise software that has never been asked the second question.
1. The Self-Marking Exam
An application writes its own history using a connection that permits updates and deletes to that history. Every log line is therefore a claim the application is making about itself, backed by nothing except the application’s continued good behavior and the assumption that nobody has stolen its credentials.
This is the failure that makes the other three possible. An attacker who reaches the application does not need to hide inside the log, and neither does a panicking employee. Either can simply amend it. The record survives precisely as long as no one has a motive to touch it, which inverts the entire purpose: the log is weakest at the exact moment it becomes valuable.
You cannot detect this from the outside either. A tampered log looks exactly like a clean one, which is the whole point of tampering.
2. The Seven-Year Promise
Ask about audit controls and you will often get a retention answer. Records are kept for seven years, archived quarterly, backed up nightly. All true, and all responsive to a different question.
Retention governs how long a record lives. Immutability governs whether it is still the record that was written. A filing cabinet that keeps documents for seven years, unlocked, in a corridor, satisfies retention completely. Vendors answer with the easy property because keeping data is a storage decision, while making it unchangeable is an architectural one that has to be made early.
3. Evidence by Screenshot
When the log cannot be trusted or cannot be produced, teams fall back on manual evidence. Priya screenshots the model settings page before the quarterly review. Someone exports a spreadsheet of who had access in March and saves it to a shared drive. A Slack thread gets forwarded as proof that a spend increase was approved.
Every one of these is an artifact produced by an interested party, after the fact, at a moment of their choosing. An auditor accepts them when nothing better exists, and marks the control as compensating rather than sound. Worse, this practice quietly teaches the organization that evidence is something assembled during audit season instead of something the system emits continuously, which means the periods nobody screenshotted are simply dark.
4. The Question With No Row
Here is where the previous three become expensive. A project budget was raised from $2,000 to $9,000 on a Thursday in February. The engagement it belonged to closed in April. In July, someone reviewing practice margins asks who approved that increase and on what basis.
The usage data answers what was spent, because consumption is the number every AI tool records well. The governance events around the spend are the ones that go missing: who requested it, who approved it, who added the provider key, who moved a person into an admin role for an afternoon and out again, who connected a project to a storage location holding client documents. Those are the actions an auditor asks about, and they are routinely the ones no system captured, because tooling is built to measure consumption rather than authority.
What an Auditor Is Actually Asking For
Strip away the frameworks and the questions reduce to three properties. None of them are about volume of logging.
1. The Writer Must Not Be Able to Reach the Record
An audit trail earns the name when the component being audited cannot alter what was written about it. The application inserts and the application’s story ends there, with no path back to amend, no path to delete, and no difference in outcome if its credentials are stolen tomorrow.
This is the same instinct behind separation of duties, applied to memory rather than to people. The person who raises the invoice does not approve the payment. The system that performs the action does not get to edit the account of it.
2. Completeness Is Judged on the Boring Events
Organizations over-log the interesting things and under-log the administrative ones. The chat transcript feels important, so it gets captured. The permission change that let someone read that transcript happens in a settings page and evaporates.
Almost every real audit question is about authority rather than content. Who was allowed to do this, who granted it, when did it change, and when did it stop. A log holding a thousand conversations and no role changes has recorded the system’s output while missing its control plane.
3. It Has to Already Be True Before Anyone Asks
Evidence gathered in response to a question is testimony. Evidence recorded at the time, by a system with no stake in the answer, is a record. The distinction decides how much work an audit is, and it is fixed long before the audit starts.
The practical version is simpler. A firm whose systems emit evidence continuously answers a client security questionnaire in an afternoon. A firm that assembles it on demand spends three weeks, every time, and gets a worse result.

Nothing is throttled and nothing is quietly swapped to a cheaper model, so there is no automated behavior to reconstruct later.
- Entries name a person, because identity comes from your directory. Access ties to Microsoft Entra ID or Google Workspace through OIDC, with domain-based provisioning and the option to enforce SSO-only login, so an entry points at a real employee in your tenant rather than a shared login somebody set up in a browser. That is what makes it evidence instead of a screenshot.
Governance behaves identically whether the tokens ran on your own provider keys or on models we host. The mode changes the invoice and nothing else, including what gets recorded.
Your firm already keeps one book it cannot edit, because a hundred years of practice established that a record the writer controls is not a record at all. AI is a system that spends money, reaches client documents, and grants access, which puts it on that list.
